PHPCN Annual Conference Call for Presenations
As part of PHPCN’s continuous effort to provide quality educational programs, PHPCN invites its
members and professionals of the palliative and hospice communities to submit presentation proposals for the
2019 Annual Conference (May 1- May 3, 2019) to be held at the Sheraton Harrisburg Hershey Hotel in Harrisburg, PA.
PHPCN Award Nominations
Nominate your colleague, community partner or volunteer for the Annual PHPCN Awards Luncheon. The luncheon will be held Thursday, May 2nd, 2019 at 1:00pm at the Sheraton Harrisburg/Hershey in Harrisburg, PA. Please submit nominations forms to the PHPCN office for the following areas:
Community Support: Recognizes an individual, business, local of state organization that has shown support of a hospice and/or palliative program that has fostered the mission and philosophy of hospice and palliative care.
Journalism: Recognizes pieces aimed at increasing community awareness of the hospice and palliative concept of care.
Heart of Hospice and Palliative Care: Designed to recognize an individual volunteer demonstrating exemplary generosity and spirit towards the hospice and palliative care community.
Spirit of Hospice and Palliative Care: Recognizes a hospice/home health aide who has demonstrated outstanding performance, sensitivity, respect and continuous excellence of service in hospice and palliative care.
The Nancy Bohnet Award for Excellence: This award was established in memory of Nancy Bohnet, both a founding member and President of PHPCN. Through Nancy's dedication she helped to create the roots of our hospice community. We are proud of this award which serves to honor Nancy's integrity as she always focused on what was best for the patient and her honesty which was apparent in all of her work. This award will honor the individual who exhibits these qualities in their everyday work.
PHPCN Annual Conference~ Joan K. Harrold, MD, MPH, FACP, FAAHPM ~ Scholarship
Apply or nominate a college for the PHPCN Annual Conference Scholarship program recognizing Joan K. Harrold, MD, MPH, FACP, FAAHPM, for her tireless efforts to make available exceptional end of life care education to the hospice and palliative care community. This is an annual scholarship providing financial assistance for a member of a hospice and/or palliative care team to attend the Pennsylvania Hospice and Palliative Care Network’s Annual Conference
Dr. Harrold is the Medical Director and Vice President of Medical Services of Hospice & Community Care in Lancaster/York, PA and has almost two decades of experience in hospice and palliative care. Board certified in Internal Medicine with added qualifications in Hospice and Palliative Medicine, she also completed a Master of Public Health degree with special emphasis in epidemiology and biostatistics.
She serves on the Board of Directors of NHPCO and previously served as a director on the Boards of the Pennsylvania Hospice and Palliative Care Network (including terms as president and vice-president), the American Academy of Hospice and Palliative Medicine (AAHPM), the American Board of Hospice and Palliative Medicine (ABHPM), and the Hospice and Palliative Nurses Foundation. She has also served as the Physician Section Leader of the National Council of Hospice and Palliative Professionals. Additionally, Dr. Harrold serves on the inaugural Examination Committee of the Hospice Medical Director Certification Board. She formerly served on the American Board of Internal Medicine Examination Committee for Hospice and Palliative Medicine and the Examination Committee for the American Board of Hospice and Palliative Medicine.
PHPCN is soliciting submissions for the Poster Session held during the Annual Conference May 1-3, 2019 at the Sheraton Harrisburg/Hershey in Harrisburg, PA.
Topic Areas for Poster Submissions:
Specific patient populations (i.e., Pediatrics, Veterans)
Integration of hospice/palliative care to specific settings (e.g. hospital, ICU, home, nursing home, clinic)
Equitable access to hospice/palliative care for vulnerable populations
Palliative Care Payment models
Psychosocial interventions and/or case studies
Intervention strategies/team based care planning
Ensuring quality in advance care planning
