Position Available:  VNA Philadelphia is seeking a full time Hospice- On Call Registered Nurse.
$2,500 sign on bonus
Job Description:  Responsibilities include providing primary RN responsive care to patients during assigned overnight and/or weekend time, completing field visits to patients to deliver needed care and complete all necessary documentation and communication related to the actions taken and issues addressed, responding to patients/families in distress and symptom management issues, follow patients to the ER when 911 is called (try to prevent admission by discussing goals of care to transfer patient to the IPU or discuss revocation.  If patient is to be admitted obtain a signed revocation form), handle new referrals (Hospice introductions and patient evaluations) in hospital, nursing facilities, or at home as needed during the off hours, tuck in visits for patients leaving the hospital, IPU or nursing facility late in the day, and time of death pronouncements.  
Requirements:  Current, valid PA RN license and specific clinical experience with Hospice Care, demonstrated clinical experience with complex acute care may be acceptable, subject to approval, and RN Diploma or BSN.

Exciting opportunity exists to join a hospital-based pain and palliative care team at Fox Chase Cancer Center.  The candidate will be involved in the supportive care of patients throughout the disease trajectory as well as patients transitioning to end of life care.
The Nurse Practitioner assesses, diagnoses, prescribes therapies, and manages the cancer patient's physiological and psychological responses to the diagnosis, treatment, and survival with cancer.  Working in a friendly, collegial, and highly collaborative model with two physicians, two NPs and a dedicated team of nurses and a social worker, the NP at Fox Chase will provide care across settings thereby enhancing continuity for the patients, families and staff.
Requirements:  CRNP in Pennsylvania; AOCN or ANP certification. 18-24 months experience preferred; oncology, pain management or palliative care experience preferred.    Must possess excellent interpersonal skills and flexibility with scheduled hours.

Job Description:
Provides and directs the provision of nursing care for hospice patients based on the agency policies and procedures through the competent application of the nursing process. Plans and implements the total palliative care of the patient in support of the prescribed medical team plan. Assists with and/or reinforces patient teaching. Provides direct nursing care, with special emphasis on control of pain and other symptoms of the hospice patient. Provides nursing services to patient/family utilizing the hospice concept.
Minimum Requirements:
Current Pennsylvania Registered Nurse licensure. Current BCLS Health Care Provider. Graduate of an approved school of professional nursing. Minimum of one year of nursing experiences. Must be able to work a holiday and weekend rotation.
Preferred Qualifications: 2-3 years' RN experience preferred. Hospice experience preferred. Hospice certification preferred. BSN preferred.
