Position Available: Advanced Practice Provider, Penn Medicine

Penn Medicine is recruiting for a full-time Advanced Practice Provider on the HUP inpatient Palliative Care consult service. The NP/PA will join our interdisciplinary team to provide recommendations for pain and symptom management, psychosocial support, and advance care planning for patients and families facing serious illness. This is a full time position, Mon-Fri from 8:30am-5pm. No on-call or weekend requirements. The ideal candidate will have 2-3 years of experience as an advance practice provider. Position Available: Palliative Medicine Physician, Penn Medicine Lancaster General Hospital

Lancaster General Health, a part of Penn Medicine, is seeking a palliative medicine physician to join a growing program throughout the health system.  There is an opportunity to work in varied clinical settings (inpatient and/or outpatient) that may be tailored to the candidate’s interests. The physician will join an already established interdisciplinary team comprised of physicians, advanced practice providers, nurses, social workers, and chaplains.  

The qualified candidate will possess broad clinical knowledge and excellent interpersonal and communication skills.  Clinical responsibilities include expert management of physical and emotional symptoms related to serious illness or disease-directed treatment, facilitation of advance care planning discussions, transition planning, and close collaboration with each patient’s primary team.  There is tremendous support from hospital leadership to expand access to high-quality primary and specialty-level palliative care across care settings through scalable models of care delivery.  The candidate will directly report to the Medical Director of Palliative Care, and will have the opportunity to participate in palliative care education and quality improvement initiatives throughout the health system.  
 
 MINIMUM REQUIREMENTS:

Must be a graduate of an accredited School of Medicine (Allopathic/Osteopathic)
Completion of an accredited residency program
Completion of an accredited Hospice and Palliative Medicine Fellowship
Completion of an accredited Possess or able to obtain an unrestricted PA Medical License
Board Certified or Board Eligible
2-3 years of experience as a Hospice and Palliative physician desirable

The Hospice Physician will join our dynamic hospice medical leadership team to assist with overseeing our free-standing 14-bed in-patient hospice facility on Black River Road in Bethlehem, Pennsylvania. This position will play a key role in advancing the mission, operations, and services of the Visiting Nurse Association of St. Luke’s Home Health/ Hospice, Inc. Summary: 

The Staff Chaplain in Palliative Care provides spiritual care in the manner consistent with the Hospital of the  University of Pennsylvania- Penn Medicine's mission, vision and values.  Committed to respecting the dignity, faith tradition, culture and/or philosophical world view of each individual, loved ones and staff, chaplains support and guide the spiritual climate of the entity. The foundation of the Chaplain services is the spirit of inclusiveness. Additionally, when requested, the Staff Chaplain will facilitate communication and connection between individuals and their religious/spiritual communities.  As an active member of the Interdisciplinary Palliative care team, the chaplain will act as a consultant to the primary clinical team, service lines and other departments. The palliative care chaplain educates students, residents, nurses, fellows and CPE interns about spiritual assessment and spiritual care generally and specifically as it relates to the care of patients receiving palliative care and their families. 

Responsibilities: 

• Provide spiritual care, evaluation and assessment of spiritual needs for patients, loved ones and staff.
• Supports the end of life care for patients by offering spiritual care support and counseling to patients and families.
• Participate in and/or facilitate patient care conferences as determined to assess patient/family understanding of illness, prognosis, goals and preferences.
• Respond to requests for spiritual support in a timely manner. 
• Fulfill regulatory requirements for spiritual assessment and documentation. 
• Act as liaison between the palliative care team and spiritual care department 
• Participate in the offering of worship opportunities, meeting ritual and sacramental needs. 
• Assist in coordinating community clergy and religious visitors. 
• Serve on institutional and palliative care departmental committees as appropriate 
• Promote an interdisciplinary approach to pastoral care. 
• Participate in maintenance of proper administrative procedures for the department in accordance with institutional policy. 
• Record spiritual care in EMR to effectively communicate with all members of the patient care team.
• Participation in inpatient consultation psychosocial team clinical responsibilities
• Participate in the department’s continuous quality improvement process.
• Contribute to the wider community through research, publication and teaching as requested. Education may include training of   medical trainees, CPE students and staff. 
• Participate in weekly palliative care interdisciplinary meetings and departmental staff meetings. 
• Participate in regular professional development activities. • Participate in regular supervision. 

Credentials:

Must be certified by and in good standing with the Association of Professional Chaplains, the National Association of Catholic Chaplains, Canadian Association for Spiritual Care, American Association of Pastoral Counselors, or the National Association of Jewish Chaplains. Certification required within 1 year of hire. 

Education or Equivalent Experience:

Master of Arts or Science in Divinity from a recognized institution (Required)
Bachelor of Arts or Science from a recognized institution (Required)
CPE in Pastoral/Spiritual Care (Required)
2+ years practice in pastoral care (Preferred)
Experience working in pluralistic setting (Preferred)
